Contract Description:
(i) This is a combined synopsis/solicitation for commercial items, prepared in accordance with the format in subpart 12.6 of the FAR and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a separate written solicitation will not be issued.

(ii) Solicitation number 18-236-SOL-00066 applies, and is issued as a Request for Quote (RFQ).

(iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-100.

(iv) This requirement is a total Small Indian Owned Business set-aside and the associated NAICS is 541519 with small business size standard of 150 employees.

Non-Small Indian Owned Business offerors will not be considered for the award.

(vi) This firm fixed price requirement is to acquire Dell/EMC hardware and software maintenance licenses for the server/storage equipment within the Indian Health Service (IHS) HQ data centers. The maintenance contrac is required for IHS to receive updates to address hardware component failures, software defects, resolve security vulnerabilities and support new features that would enhance the capability of the equipment. In addition, the maintenance contract shall provide IHS with next-day replacement equipment in the event of a hardware failure as well as technical support directly from systems engineers that support the equipment.

(vii) Delivery and acceptance terms for this order is FOB Destination. Delivery Addresses: Indian Health Service, 5600 Fishers Lane, Rockville, MD 20857; and 1011 Indian School Road, Albuquerque, NM 87104.

(viii) The provision at 52.212-1, Instructions to Offerors – Commercial Items (Aug 2018) applies to this acquisition.

(ix) 52.212-2, Evaluation — Commercial Items (Oct 2014) The award will be made to the lowest priced technically acceptable offeror. Acceptable Business Quote shall include a completed Price Table provided via Attachment 1. To be considered technically acceptable for the award, the offeror shall provide a reseller/partnership certificate that evidences the company’s eligibility and capability to provide the requested Dell EMC products. Additionally, the offeror shall provide the following information for two relevant federal contract awards the offeror received within past 4 years : Product description, project magnitude, outcome and customer contact information. Only exact “brand name” products and part numbers shall be acceptable with the offer. No “identical” or “like” products and part numbers will be accepted.

(x) Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications — Commercial Items (Aug 2018), with its offer.

(xi) FAR 52.212-4 Contract Terms and Conditions – Commercial Items (Jan 2017) applies to this acquisition. The following addendum applies: Email Addresses for submission of invoices are paul.premoe@ihs.gov, COR’S, and HQInvoices@ihs.gov.
